Prior to departure from the DOC facilities, all Affiliates must complete:

• CD-126 (DAO 207-1)

For all new personnel, the following paper work is required:

• Security Coversheet / Investigation request / Badge request
• FD-258 (fingerprint card) or SF-87 for Federal Employees
  
• OF-306 Declaration for Federal Employment

 After favorable prints comes back, the individual will be invited into E-QIP to finish the investigation or a SAC will be completed

PIV ID Badge Requirements:

• Access Request Form (Boulder Labs Only)

Personnel in Low Risk Positions who will be in DOC Facilities for 10-180 days must complete:

• OFI-86C
• FD-258 OR SF-87

Personnel in Low Risk or Moderate Risk Positions who will be in DOC Facilities for more than 180 days must complete:

• SF-85 (Must be submitted electronically through E-QIP. Contact your HR representative or Rebekah Lamoree  (303-497-3943) in the Security Office for further details)
• FD-258 or SF-87
• Resume OR OF-612 (Not Required for Contractors)
• OF-306

Personnel in Moderate Risk ADP Positions must complete:

• SF-85P MBI (Must be submitted electronically through E-QIP. Contact your HR representative or Rebekah Lamoree  (303-497-3943) in the Security Office for further details)
• FD-258 OR SF-87
• Resume OR OF-612
• OF-306
• Fair Credit Reporting Act of 1970 Form
  

Personnel in High Risk Positions must complete:
